OTOSCLEROSIS

Otosclerosis is called heardning of the ear and over growth of the sponge bone of middle ear (stepis bone) that lead to sponge bone become fixed, which seprate the middle ear & inner ear so that sound can not conducted to inner ear. Due to immobalization of the stepis bone which reduce the transmission of vibration to the inner ear.

Insedence - More in female than male.

 

Etiology :

                -  Idiopathic (Any disease with an unknown cause or mechanism).

-        Heredity (आनुवंशिक )

-        Race (white are more affected than negros)

-        Age (more in b/w 22-30 years)

 

Clinical manifestation :

-        Tinituss ( कान में घंटी या सीटी की आवाज आना )

-        Irritability

-        Hearing loss

-        Lack of response to conversatation

-        Restlessness

Pathophysiology :

                               Due to etiological factors

                                                ↓

                      Abnormal growth of spong bone

                                                ↓

                        Fixcetion of the stapes bone

                                                ↓

Reduce the transmission of the vibration to the inner ear

                                                ↓

                                        Hearing loss

                                                ↓

                                         Otosclerosis

Diagnostic evaluation :

-        History collection

-        Physical examination

-        CT Scan

-        Weber & rinee test

-        Audiogram

 

Medical management :

•Medication such as oral chloride, calcium or vitamin-D may help to control the hearing loss.

•Hearing aid may be use to treat hearing loss.

•This disease will not cure or prevent or hearing loss but may help relieve symptoms.

 

Surgical management :

Stepidectomy –

Stepidectomy consist of removing of a portion sclerotic part of the stapes bone and replacing it with an implant or prosthesis that is placed between the incus and ovel window.

Stepidotomy –

A modern surgery called stepidotomy is perform by drilling a small hole in the stapes bone with micro drill or a laser and the insertion of pistel like prostheses.
