TONSILLITIS

It is the inflammation of the tonsil. The tonsil are compose of the lymphatic tissue which are situated each side of back of throat.

The tonsil act as a filter and also produce antibodies to fight infection but some time tonsils become infected this condition is known as tonsillitis.

Types –

              (i)  Acute

              (ii) Chronic

Acute – Bacteria or virus infects the tonsil causing swelling and a sore throat.

Chronic – Persistent infection of the tonsil, some time as a result of the repeated episodes of tonsillitis.

 

Etiology –

-        Most common cause bacteria & virus infection (streptococcus)

-        Other causes adenovirus, influenza virus, epstinbaar virus, herpis simplex virus

 

Pathophysiology –

Due to etiological factors

Invision of causative agent into the tonsil

Infection occur

Antigen antibody reaction

Histamine & prostaglandin release

 ↓

Inflammation

 ↓

Clinical manifestation –

-        Pain

-        Snoring

-        Sleep apnea

-        Throat pain

-        Headache

-        Loss of appetite

-        Anorexia

-        Difficulty in swallowing

-        Fever

-        Enlarge red tonsil

-        Nausea & vomiting

-        Hoarseness sometime abdominal pain

-        Trismus (difficulty in opening mouth

 

 

 

 

 

Diagnostic evaluation –

-        History collection

-        Physical examination

-        Blood test

·      CBC 

·      ESR

·      Blood sugar

-        Throat swab test

 

Management -

Medical management -

•Antiboitic -  Amocixilline

•Analgexices -    Dixfenic

•Antipyretic  -   PCM

•Antihistamin  -  Livocitrizine

 

Surgical management –

Tonsillectomy – Removal of the tonsil.

 

 

 

Complications -

* Pharyngeal abscess

* Peritonsilar abscess

* Tonsiler cellulites

* Obstructive sleep apnea
